Through years of experience 99.9% of insurance companies do not cover medical weight loss services or products. Therefore, at Dr. Rogers Wellness and Weight Loss Center we do not accept any insurance reimbursement as a form of initial payment for medical weight loss. We will as a courtesy, bill your insurance for your initial visit and once insurance has declared the coverage status of your claim then the appropriate payment plan will be established.

HMO’s - If you currently participate with an HMO insurance, then follow these steps:
		1) Request a referral from your primary care physician. 2) At a minimum, request 8 visits to cover 6 months of treatment. 3) Have your primary care physician's office fax the referral to (888) 893-4363. 4) Once our team receives your referral, we will call you to schedule an appointment. 5) If you are having trouble with the referral, don’t hesitate to call.
