Bio-Identical Hormone Program

Dr. Kyla Helm-Swanson and Kim Diaz, P.A. specialize in the treatment of bio-identical hormone replacement for both men and women. The initial Bio-Identical Hormone evaluation is scheduled for an hour and a half secondary to the complexity of signs and symptoms of hormonal imbalance. Follow up visits are scheduled for an hour and usually occur every 2 to 3 months to establish hormone balance.

Initial Visit - $250

Follow-Up Visit - $150

Bio-Identical Hormone visits are considered to be cash pay at the time of service and all of the necessary documentation to file claims with your insurance provider, flexible spending accounts or health finance accounts will provided at the time of service. Dr. Kyla Helm-Swanson is considered an out of network provider because she does not participate with any insurance companies.
